At the core of MEDecision's Integrated Medical Management solution, CarePlanner Web leverages integration of MEDecision's Analytics and Disease Management to identify patients needing to be managed within particular disease management plans, enabling MCOs to create the right programs and provide appropriate feedback to providers to maximize patient care.
Once the patients are identified for management, CarePlanner Web can document and manage specific contacts for case management. The system can record and document the next-door neighbor for a frail, elderly patient, so the case manager can call the neighbor to check on the patient rather than contacting 911 emergency services.
Combining Predictive Capabilities with Clinical Management
With CarePlanner Web, documented patient information derived from claims data can be used to pre-populate questionnaires case managers use to interview patients and providers. In addition to increasing administrative efficiencies by eliminating manual data entry, pre-population drives higher levels of care to patients by generating integrated action plans tailored to each patient. Additionally, CarePlanner Web enhances the analytics process within IMM by assessing questionnaire answers and then generating a list of nursing problems for which case managers should be managing, thereby improving the quality of care by ensuring a consistent process across all patients.
As an Integrated Medical Management component, CarePlanner Web delivers an integrated letter engine for better facilitation of automated correspondence.
